Inland Revenue form 64-8 which will enable us to correspond with the Inland Revenue on your behalf. Please sign.
Tax information request to complete your 2009/10 tax return. [If this may take time to collect then this can be forwarded later]
A personal questionnaire for completion.

Also if you work on a diver support, construction, pipelaying vessel or related could you provide details of the type of ship that it is. Please refer to our news page for the issues involved here.
